HOW TO USE
• Open the PDF in Adobe Acrobat Reader (free) or another PDF app that supports fillable forms

• Keep the original file blank and start from it for each child
• Some browser and phone PDF viewers do not save form fields - a desktop PDF reader works best

• Standard 20-24 lb (75-90 gsm) paper works well
